How Does USP Suture Sizing Work?

The United States Pharmacopeia (USP) suture sizing system uses a numerical scale where larger numbers with a zero suffix indicate smaller sutures. For example, 5-0 is thinner than 3-0, which is thinner than 0. Above 0, sizes increase as 1, 2, 3, etc., indicating progressively thicker sutures. The system dates to the early 1900s and, despite its counter-intuitive nature, remains the global standard.

USP to Metric Diameter Conversion

The European Pharmacopoeia (EP) uses a metric system based on actual suture diameter in millimeters. Key conversions:

  • USP 7-0 — Metric 0.5 — Diameter 0.050–0.069 mm
  • USP 6-0 — Metric 0.7 — Diameter 0.070–0.099 mm
  • USP 5-0 — Metric 1 — Diameter 0.100–0.149 mm
  • USP 4-0 — Metric 1.5 — Diameter 0.150–0.199 mm

Which Size for Which Procedure?

  • 6-0 to 7-0 — microsurgery, ophthalmic, pediatric vascular.
  • 4-0 to 5-0 — face, hand, subcuticular closure, vascular anastomosis.
  • 3-0 — general skin closure, muscle, peritoneum.
  • 2-0 to 0 — fascia, abdominal wall, orthopedic soft tissue.
  • 1 to 2 — heavy fascia, sternotomy, large tendon repair.
